New Patient Information
• First visits require 1 hour.
• Please request 2 appointment slots back-to-back to ensure your visit is approved.
• Scheduling Online: You must receive a confirmation notice to verify your appointment.
How to Schedule
• Request online via our appointment form.
• Ensure you receive a confirmation notice to verify your appointment was accepted. Otherwise, you do not have an appointment.
• New patients require 1 hour for the first visit. Please request 2 appointment slots back-to-back; otherwise, it will not be approved.
Insurance & Billing
• First visits require 1 hour.
• Currently credentialed with Blue Cross & Blue Shield (BCBS).
• Our lab is credentialed with all insurances.
If you are insured with a provider other than BCBS, you are responsible for the office visit; however, labs are billed to your insurance.

- Hormone Replacement Therapy
- Pap Smears
- PrEP
- STI Screening & Testing
- Primary Care Provider for established Trans patients
We currently accept Blue Cross and Blue Shield and offer a self-pay option. At the moment, we are only able to see patients who are 18 and older.

Established patients can submit all prescription requests through your pharmacy. If you have any questions or concerns, please feel free to call us for assistance.
Appointments must be made at least 24 hours before your requested appointment time.
