We currently accept Blue Cross and Blue Shield and offer a self-pay option. At the moment, we are only able to see patients who are 18 and older.

You can submit all prescription requests through your pharmacy or, if you have trouble, you can call us with questions or concerns.
HOURS
Tuesdays - 9 am to 5 pm
Wednesdays - 9 am to 5 pm
Fridays - 1 pm to 4 pm
Appointments need to be made 24-hours prior to your requested appointment time.
